Primary Responsibilities:
The Adult Services Division provides services to individuals with ID/DD and Autism living in a variety of settings. Each individual is assisted in establishing and maintaining a home that is safe, secure, and comfortable. The program strives to maintain the stability of the person’s home by increasing or decreasing services as appropriate. The RN will be a compassionate, professional member of each individual’s team to provide medical consultation and supports. Duties include accompanying individuals and their staff to medically complex appointments; assess, plan, implement and evaluate individual’s medical care plans; communicate with and/or prepare other staff to communicate with medical providers; interpret and evaluate diagnostic tests to identify and assess individual’s condition; monitor individual’s medical care plan including diet and physical activity; instruct individuals and staff on topics such as vital signs, and health education and disease prevention. This is a 20 hour per week position on site within program operating hours.
Minimum Qualifications:
· Must possess a current, active license as a registered nurse under the MA Board of Registration in Nursing
· Experience in the care of individuals with a wide spectrum of diagnoses
· Familiarity with Electronic Healthcare Record Systems and HCSIS helpful
· Familiarity with Medication Administration Program (MAP) helpful
· Proficiency in MS Office Suite, Power point, and MS Outlook
· Demonstrated ability to efficiently balance concurrent and changing priorities

Compassionate demeanor with a strong commitment to patient advocacy.
· Acceptable driving record, valid driver’s license and access to an automobile suitable to transport individuals if needed
